Аутосомно-рецессивный врожденный ихтиоз. Клинический случай: ихтиоз по типу «купального костюма». От фенотипа к генотипу
КЛИНИЧЕСКИЙ СЛУЧАЙ
Аннотация
Ихтиозы — генетические заболевания, характеризующиеся генерализованной сухостью кожи, шелушением и гиперкератозом, нередко сопровождающиеся эритродермией. Согласно итогам консенсусной конференции по ихтиозу 2009 г. различают синдромальные и несиндромальные формы заболевания. Аутосомно-рецессивный врожденный ихтиоз относится к несиндромальным формам ихтиоза. Клинически аутосомно-рецессивный врожденный ихтиоз подразделяется на три основных фенотипа: ихтиоз Арлекина, пластинчатый (ламеллярный ихтиоз) и врожденная ихтиозиформная эритродермия. Реже встречаются менее распространенные формы: самоизлечивающийся коллоидный ихтиоз, акроакральный самоизлечивающийся коллоидный ихтиоз и ихтиоз по типу «купального костюма». Одним из наиболее легких вариантов аутосомно-рецессивного врожденного ихтиоза является ихтиоз по типу «купального костюма», который характеризуется шелушением кожи с локализацией преимущественно на туловище без поражения конечностей и лица. Заболевание вызвано чувствительными к температуре вариантами трансглутаминазы 1, кодируемой геномTGM1. Описано клиническое наблюдение ребенка трех месяцев с аутосомно-рецессивным врожденным ихтиозом по типу «купального костюма». Пациент родился недоношенным с универсальным поражением кожи по типуколлоидной пленки, которая отшелушилась в течение 10 дней. Из родильного дома новорожденного перевели Детскую городскую больницу № 1. Системную терапию ребенку не проводили, наружно применяли мазь с декспантенолом. При генетическом обследовании у мальчика выявили ранее не описанный как патогенный вариант мутации в экзоне 3 генаTGM1.В возрасте трех месяцевродители ребенка обратилисьв консультативно-диагностический центр Санкт-Петербургского государственного педиатрического медицинского университета, при его осмотре обратили внимание на своеобразную картину поражения кожи. Старшая сестра пациента также страдает врожденным ихтиозом с характерной локализацией поражения кожи на туловище и волосистой части головы. Продолжение поиска новых этиологических факторов аутосомно-рецессивного врожденного ихтиоза расширит наше понимание как патогенеза ихтиоза, так и может быть полезно с точки зрения поиска новых терапевтических подходов к лечению этого заболевания.
